ABSTRACT

 

Rheumatoid arthritis is a chronic inflammatory disorder with an autoimmune pathology affecting variety of body systems primarily the joints. It affects the lining of joints, causing painful swelling that can eventually result in bone erosion and deformity. Due to similar presentations Rheumatoid arthritis can be correlated with Amavata mentioned in our classics.

In Ayurveda principles of treatment of amavata are langhana, swedana and drugs having tikta, katurasa, deepana, virechana, snehapana and vasti. Vasthi karma is the best therapy to treat all the vatavyadhi . In pain and swelling of Rheumatoid arthritis nirooha vasthi which have the agni deepana , pachaka, vatakaphahara and shoola sothahara properties are essential , Rasnadi vasthi mentioned by chakrapani commentary on charaka Samhitha is one among them.

Objective of this study is to evaluate the effect of Rasnadi vasthi administered continuously for 7 days in Rheumatoid arthritis patients. The study will be a before and after study. Participants of both sex aged 30 to 60 registered in IPD of Government Ayurveda Panchakarma Hospital, Poojappura, Thiruvananthapuram diagnosed to have Rheumatoid arthritis and satisfying inclusion criteria will be selected. The base line data will be collected through interview, observation and questionnaire. After collecting base line data, detailed history and examination will be done using clinical case proforma.

 

A total of 20 participants will be selected by consecutive sampling and Rasnadi vasthi will be done continuously for 7 days. The participants will be assessed before and after the intervention (ie, on 0th, 8th Days) with regards to changes in DAS 28 Scale and VAS. Data will be Statistically Analysed and Presented.

DISSERTATION TITLE: EFFECT OF SAMANA SNEHAPANA WITH SAHACHARADI TAILA IN COMPARISON TO DHANWANTARAM TAILA IN THE MANAGEMENT OF OSTEOARTHRITIS OF KNEE JOINT

Osteoarthritis of knee joint is a common progressive multifactorial joint disease and is characterized by chronic pain and functional disability. OA is the second most common rheumatologic problem of 22% to 39% in India. OA is common in women than men, but the prevalence increase dramatically with age. In Ayurveda OA knee may be well considered as Sandhigata vata a type of VataVyadhi affecting the sandhi, explained by all the acharayas. As vata is predominantly vitiated, Snehanam is one of the effective treatment to treat vitiated vata. Among the snehana therapy, Samana snehapana could be very effective treatment modality. Sandhigata vata / OA knee is a condition where cartilage in the knee degenerates results in bone rubbing together and creating stiffness, pain and impaired movements. Sahacaradi taila, in Astanga hridaya, Astanga sangraha which is mentioned in Vatavyadhi chikitsa and it is very effective in adha – kaya vata rogas. The objectives of this study is to compare the effect of samana snehapana with Sahacaradi taila and Dhanwantaram taila in the management of OA knee. The study is comparative clinical trial with control group. Participants of both sex, aged 40 – 60 years registered in OPD and IPD of Government Ayurveda college Panchakarma Hospital, Trivandrum satisfying the inclusion criteria will be subjected to consecutive sampling and the total of 32 participants will be selected for the study. The data will be collected through observation, clinical case proforma and X-ray. After collecting baseline data, detailed history and examination will be done using clinical case proforma. A total of 32 participants will be selected for the study. They will be subjected to consecutive sampling, divided in to 2 groups (Group A and Group B). In Group A Sahacaradi taila as samana sneha pana will be given for 14 days and Group B, Dhanwantaram taila as samana snehapana will be given for 14 days. Both the groups will be assessed before and after the intervention on 0th day, 6th day, after treatment on 20th day, followup on 35th day and 65th day. The result will be analyzed statistically by using Descriptive and „Mann Whitney U Test‟.
