ABSTRACT

Otomycosis is a fungal infection of the ear canal. That often occurs due to Aspergillus niger, A.fumigatus or Candida albicans. It is characterized by intense itching, discomfort or pain in the ear, watery discharge with musty odour and ear blockage. Its prevalence has been quoted to range from 9% to 27.2% among patients who present with signs and symptoms of otitis externa and up to 30% in patients with discharging ears . Otomycosis shares symptomatic similarity with karnakandu and kaphaja karnasoola in Ayurveda classics with a predominance of kapha and vata . Ingredients of Nirgundyadi taila are Kapha-Vata samana and possesses properties like kandughna,krimighna,and kushtaghna . Pichu vartis are medicated cotton wicks. Which are highly beneficial in the treatment of dustavranas . According to Vagbata Nadivrana and dustavrana chikitsa can be adopted in karnarogas with discharge, pain and 2 feeling of heaviness. Application of pichuvarti is also mentioned in the treatment of karnarogas manifesting with pus discharge, for clearing the pus and applying medicine to ear canal.

Study design is pre and post interventional study with sample size 21. Participants of the study having signs and symptoms of Otomycosis satisfying inclusion criteria were selected from OPD of Shalakyatantra, Govt Ayrveda college Thiruvananthapuram.The study period is 18 months. Pichuvarthi soaked in nirgundyadi taila inserted in to the affected ear canal after thorough ear toileting and retained for 45 min. The procedure repeated for 14 consecutive days and assessment done before treatment, after treatment and during follow up that is on 46th day (1month). Assessment was done on the basis of signs and symptoms of Otomycosis. At present 12 participants completed the study and effect of therapy was also assessed among them. The final result will be statistically analyzed with Wilcoxon signed rank test after completing the study.

ABSTRACT

Rasasastra is the branch of Ayurveda deals with processes by which various metals, minerals and other substances including mercury are purified, combined with suitable herbs in appropriate ratio in an attempt to treat illness. From 5th century onwards, the advancement rasasastra and use of metals or minerals opened a new frontiers for ayurvedic pharmacology and pharmaco-therapeutics. But the toxicity of traditional ayurvedic medicines with metalomineral combinations is a matter of concern among ayurvedic practitioners due to the presence of heavy metals. Rasaushadhis irrespective of its lesser therapeutic doses, enhancement of action of other ingredients in the formulation, quicker action and wide range of therapeutic purposes are always questioned for its safety profile. Suryaprabha gulika is a traditional ayurvedic formulation mentioned in Sahasrayogam containing both herbal and mineral ingredients and is indicated for the management of fever and upper respiratory tract infections. Though it is available in our market, there is no scientific data available regarding the therapeutic index of the formulation. This study aims to evaluate the acute and subacute toxicity of the formulation in wistar albino rats of either sex. Animals will be sacrificed at the end of the study. Physiological, haematological, biochemical and histopathlogical parameters will be assessed thereafter. Datas obtained after the study will be statistically analysed using appropriate modes.

ABSTRACT

Nausea and vomiting is a common experience in pregnancy affecting 70 to 80% of all pregnant women. Altered immunological hormonal and psychological states are responsible for initiation of symptoms include nausea vomiting retching and occasional sickness. Simple vomiting if not properly managed with active antenatal care which may cause hyper emesis gravidarum which in turn cause slow starvation dehydration electrolyte imbalance weight loss and lead to impairment in daily activities of pregnant lady. Anti-emetic and anti-histamines are used in conventional Allopathic practise. In unresponsive cases need hospitalization and even termination of pregnancy. So, there is an intense need of alternative nontoxic safe and economic remedy to manage this condition.

            In Ayurveda all the classical texts mentioned nausea and vomiting are symptoms of normal pregnancy. Acharya Hareetha included garbhini chardi among garbha upadravas which indicates any disorder of garbhini due to foetus. Considering sukha sukumara properties of garbhini, Malarinji modaka can be judiciously given. Malar (laja that is the puffed rice) is the agrya aushadha for chardi and inji (ginger) has proven effect on nausea and vomiting in pregnancy. The study design is a non-randomized control trial. 52 participants below 22 weeks of gestation with PUQE score between 6 and 13 selected from the OPD of Govt. Ayurveda College Hospital For Women and Children Poojappura. Participants are allocated to Group A and Group B. Group A is the trial group in which Malarinji modaka is given to participants, Group B is the control group in which Doxinate (Doxylamine and pyridoxine) is given to participants for treatment. Change in frequency of nausea and vomiting is assessed with PUQE score. Results statistically analysed, summarized and conclusion would be done.

ABSTRACT

Pregnancy and child birth produces a variety of physiological, psychological & socialconsequences. Postpartum depression is the major psychiatric illness among them. Prevalence estimates ranges from 13-19% in resource rich setting and 11-42% in resource limited setting. Postpartum depression has a significant impact on the mother-infant relation, in the family and with her partner. The exact cause of postpartum depression is not detected. It may be due to the abrupt change in reproductive hormonal level which rises during pregnancy. Early identification and management of postpartum depression is very essential, otherwise it may lead to a condition of depression even after the period of postpartum. Usually antidepressant medicine and psychiatric counselling are used to treat postpartum depression conventionally, but side   effects of antidepressant medicine badly affect the lactating mother and also the infant. In Ayurveda we can’t see a direct correlation of postpartum depression. Vishada can be correlated with this condition. Kushmanda swarasa gritha is chosen as a trial drug which is described in Apasmara chikitsa by Acharya Vagbada. It is a good Medhya preparation, cost effective and also easily available.  

Study design is pre and post interventional study with sample size 22. Participants of the study are postpartum ladies who are diagnosed with postpartum depression by Edinburgh post natal depression scale selected from the OPD of Prasuthi Tantra and Streeroga, women and children hospital Poojappura Thiruvananthapuram. Participants who satisfy inclusion criteria are selected. Study tool is Edinburg postnatal depression scale, Case Performa and prepared questionnaire. The study period is 18months. Prepared Medicine will be dispensed through OPD in a 200g container and instructions will be given to use daily 24 g of two equal divided doses in empty stomach with hot water as anupana for 30 days. The Edinburgh postnatal depression scale of the participants will be reassessed on 31st day and compared with previous score.

ABSTRACT

Overweight and Obesity are defined as an abnormal fat accumulation that may impair health. Obesity is expressed in terms of body mass index (BMI). WHO says a person with BMI of 30 or more is said to be obese and is a major risk factor for a number of chronic diseases.

 In Ayurveda it can be considered as Sthoulya. According to Charaka Acharya, Udwarthana is one among the treatment modalities of sthoulya. Udwarthana is kaphamedo vilayana, stireekarana of angas and twak prasadanam. Udwarthana is classified into two by acharya Susrutha with relation to mode of application as Udgharshana and Utsadana. Utsadana gives lovely apperance for the females, praharsha, soubagya,mrija and laghutwa. Thriphala is Rasayana , it cure skin diseases and also have kaphamedohara property. Objective of this study is to compare the efficacy of these two treatments in obesity.

Patients of both sexes aged 25-65, with obesity attending OPD and IPD of Govt. Ayurveda College Panchakarma Hospital, Poojappura are randomly selected as study population. They are divided into two groups namely group A and group B. Then group A is subjected to Udwarthana for 14 days and group B is subjected to Utsadana for 14 days. Both the groups were assessed before and after the treatment(ie.0th ,15th, 21st,28th day) with regards to changes in BMI, waist circumference, bodyweight, waist to hip ratio and skin fold thickness.Data will be analyzed according to statistical test.

ABSTRACT

Microalbuminuria (MAU) is defined as levels of albumin ranging from 30-299 mg in a 24-h urine collection and is an important predictor of Diabetic Nephropathy. Diabetes Mellitus has become an ever-growing world-wide health problem in all socio-economic groups. India currently represents 49 percent of the world’s diabetes burden, with an estimated 72 million cases in 2017, a figure expected to almost double to 134 million by 2025. The increasing incidence of the disease elevates its complications ranging from acute, life-threatening conditions such as severe hypoglycemia or ketoacidosis to chronic, debilitating complications affecting multiple organ systems such as retinopathy, nephropathy, neuropathy and cardiovascular disease. Out of these, Diabetic Nephropathy (DN) occuring due to microangiopathies is definitely on a rise. The earliest clinical evidence of Diabetic Nephropathy is Microalbuminuria (MAU) as defined as 30-299 mg/d in a 24 –hour collection. Since MAU is the prime marker for early suspection to renal affliction, the need of studies with regard to reducing MAU and maintaining glycemic control becomes important.

The drug of study is Gokshuradi Guggulu mentioned in YogaratnakaraMehaadhikara, indicated in conditions of prameha, vataroga, vatasonita, mutraghata, mutradosha and pradara.  In addition, the ingredients are primarily found to be pramehahara and rasayana with special affinity to vasti and mutravahasrotas. The study is intended to evaluate the effect of the drug in reducing microalbuminuria associated with Type 2 DM. The trial drug is administered in a dose of 2 gutikas of 500 mg each thrice a day, half an hour before food with lukewarm water for a period of 30 days. A sample size of 23 patients aged between 35 to 65 years with known Type 2 Diabetes Mellitus and urine albumin within the range of 30-299 mg in 24-hr urine sample are selected according to inclusion and exclusion criteria. Level of urine microalbumin in 24-hr urine sample is noted before the study and after one month on the 31st day. The data collected will be statistically analysed.

Attention Deficit Hyperactivity Disorder is the most commonly diagnosed childhood disorder, characterized by extreme and persistent restlessness, sustained and prolonged motor activity, difficulties in sustaining attention and impulsivity. The present study aims to analyse combined effect of Tiktaka gritha and Yashtichurna in children with Attention Deficit Hyperactivity Disorder and to find out the improvement in Theory of Mind among them. The cases will be recorded using socio-demographic and clinical data sheet.   Children’s hyperactivity/inattention measured using ADHDT-2 scale and Theory of Mind will be assessed using Theory of Mind task battery. To evaluate the effect of Tiktaka gritha and Yashtimadhu churna in ADHD and Theory of Mind, a sample of 12 children with age group of 5–12 years fulfilling the inclusion criteria and diagnostic criteria will be selected from Government Ayurveda hospital for women and children Poojapura OPD.  Trial drug will be given for sixty days. Before and after treatment, data will be collected, scored and statistically analysed using appropriate statistical technique.

ABSTRACT

Cosmostigma racemosum(Roxb.)Wight is a green milky weed climber belonging to Asclepiadaceae family. The present study is conducted to evaluate the wound healing activity of the plant. The descriptions from Hortus Malabaricus and various ethnobotanical studies of the sacred groves of temples in Kannur district serve as the base of this study. Wound healing is a complex process, which involves haemostasis, inflammation, proliferation and remodelling with scar tissue formation. The mobilization and proliferation of fibroblasts and angiogenesis play a major role in wound healing. A potent wound healer aids the body tissues in these steps more effectively by reducing the healing time .

The pharmacognostical, physical and phytochemical analysis of Cosmostigma racemosum(Roxb.) Wight are done as these details are not yet scientifically validated. In the in vitro assay, the cell mobilization of fibroblasts will be monitored by Scratch assay of L929 cells. Angiogenetic potential of the drug will be assessed using CAM Assay model (Chick Chorioallantropic membrane model). The wound healing activity of the plant is studied in vivo in Wistar rats by Excision wound model. Excision wounds of approximately 450mm2 will be made on the dorsal side of pre anesthetised rats. Three groups containing six rats each are used for study. For group I topical serveas the negative control.For group II application of Povidone iodine is used which serve as the standard. For Group III topical application of leaf paste of Cosmostigma racemosum (Roxb.)Wight is used which serve as the test group. The application is once daily for 14 days. Assessment will be done on 0th , 2nd ,4th ,6th ,8th ,10th and 14th days. The rate of wound contraction, period of epithelisation and histopathological analysis are assessed by this model.
