ABSTRACT

Urolithiasis is formation of calculi anywhere along the course of urinary tract. Urolithiasis can cause post- renal acute kidney injury via obstruction of urinary outflow often set off end stage renal disease (ESRD). Extracorporeal shockwave therapy (ESWT) is the treatment of choice which is not conducive because of side effects (internal bleeding, kidney damage) and high rate of recurrence (about 60% within ten years). There is a scope to find out a medicine which is effective with less side effect. Ashmari is one among the ashta mahagada.So the early diagnosis and proper treatment in proper time is very important in it.In the present study an effort is made to evaluate the Effect of Combination of kshara (Ash) of bark of varuna (Crataeva nurvala Buch – Ham) and yava (Hordeum vulgare Linn) along with guda (Jaggery) and ghrita(Cow’s Ghee) in Urolithiasis. The study drug varunadi kshara yoga is mentioned in Vangasena Samhita Asmari rogaadhikara. Kshara chikitsa is mentioned in treatment of ashmari. Study drug is easy to administer and if proven by clinical trial, drug can be used in the better management of urolithiasis.The trial drugs were subjected to preliminary phytochemical analysis.This study assess the the Effect of Combination of kshara (Ash) of bark of varuna (Crataeva nurvala Buch – Ham) and yava (Hordeum vulgare Linn) along with guda (Jaggery) and ghrita(Cow’s Ghee) in Urolithiasis  in patients with size of stone between 2mm to 10 mm determined by Ultra Sonography of abdomen.Participants of age group of 20 to 65 years were selected as per inclusion criteria and exclusion   criteria. Primary outcome variables size of stone and secondary outcome variable like routine urine analysis (R.B.C, pus cells) and assessment criteria score were analysed before and after treatment. The pre and post data test data will be analysed statistically using paired t test and Wilcoxon ranked t test. Study concludes that the trial drug combination of kshara of bark of varuna (300mg) and yava (150mg) with ghrita(75mg), guda (75mg) administered once daily for 30 days is effective in reducing stone size and symptomatic relief of participants.

ABSTRACT

Alopecia areata is a common autoimmune disorder that often results in unpredictable hair loss. It affects 2% of the World population. In India, the prevalence of Alopecia areata is 0.7%. Eventhough it is not a life threatening disease, it is associated with beauty and youthfulness in women and men.Use of potent corticosteroids is the most effective therapy in modern medicine, although overuse these produces side effects & may lead to skin atrophy.So the remedies in Ayurveda have more acceptance.In Ayurveda, signs and symptoms of Alopecia areata can be corelated with Indraluptha. Acharya Vagbhata explained Indraluptha under sirorogadhikara and Acharya Susrutha under Kshudrarogadhikara. Among Panchakarma, Nasyakarma is considered as best therapeutic measure for the disease of urdhvajatrugatha pradesha. Pratimarsha nasya is a special type of sneha nasya with low dose & minimal complications. Madhookayashtyadi ghritha is one of the herbal formulations prescribed in Ayurvedic text Ashtanga Hridaya Shirorogadhikara. 
The study design was pre and post assessment interventional study with a sample size of 20 participants with an age group 20 to 60 years who were diagnosed having Alopecia areata attending OPD& IPD of Panchakarma and Kayachikitsa at Govt.Ayurveda college, Thiruvananthapuram. Participants were administered Pratimarsha Nasya with Madhookayashtyadi ghritha for continuous 21 days, twice daily morning 8 AM & evening 5 PM, with a dose of 1 ml in each nostril. Assessment of the study was done using SALT Score. The pre and post test data was analysed statistically using paired t test. The study concludes that Madhookayashtyadi ghritha pratimarsha nasya is effective in changing the SALT score of participants having Alopecia areata.

ABSTRACT

Hyperuricemia is defined as a serum urate concentration of >6.8mg /dl for men and >5.7mg/dl for women. Worldwide, the prevalence of hyperuricemia has increased substantially in recent decades. The most well-known disease induced by hyperuricemia is gout. However, many studies have reported that   hyperuricemia also plays important roles in cardiac-kidney vascular system disease and metabolic syndrome. Hyperuricemia results from the increased production or impaired excretion of metabolic product of purine metabolism, uric acid4. Since it is an outcome of impaired uric acid metabolism, in Ayurveda this clinical entity can be correlated to Kapha- Pitta pradhana mala sanchaya in the level of rasa-rakta dhatu attributed by rasa –rakta dhatwagni mandhyam. The study drug Navakarshika kwatham described in Chakradutta Vatarakta chikitsa shloka no23-25 is widely used by Ayurvedic practitioners of kerala for treating hyperuricemia and its complications and is a unique combination of ama pachana, agni deepana, rakta prasadana, kapha-pittahara drugs. In this study clinical evaluation of hypouricemic effect of Navakarshika kwatham will be done as interventional study-pre and post Test without control group in 20 patients. The study setting is OPD of Kayachikitsa, Government Ayurveda College Hospital,Thiruvananthapuram. Patients in the age group of 30 -60 years, males having serum uric acid levels above 6.8mg/dl and females having serum uric acid levels above 5.7mg/dl will be included, drug Navakarshika kwatham administered for 30 days and clinical assessment will done by estimation of serum uric acid levels before and after the intervention.
