ABSTRACT

Desa refers to the natural inhabitancy of an individual. It plays a major role in the manifestation and prognosis of diseases. Anoopa desa is considered as the worst desa for living with prevalence of many diseases like sleepada, galaganda, apachi, jwara, amlapitta etc. Amlapitta is a pathophysiological condition in which the pitta gets vitiated in terms of vridhi and amlatha. The cardinal signs and symptoms of the disease explained in classics are avipaka, aruchi, tiktamlodgara, hriddaha, kandadaha, gourava, utklesha and klama. Acharya Kasyapa has specifically mentioned prevalence of amlapitta in anoopa desa.

Geographical features of Thakazhy grama panchayat like many lakes and rivers, heavy rainfall, marshy area, scanty breeze etc., closely relates with the characteristics of anoopa desa depicted in Ayurveda classics. Hence the primary objective of the study was to find out the prevalence of amlapitta in Thakazhy grama panchayat.

Study was done in three steps. In the first step a thorough literary review on amlapitta, acid peptic diseases and desa were done. In the second step a semi structured questionnaire was prepared. Face validation, content validation and pilot study was also done. In the third step with the help of this questionnaire a house to house survey was carried out among 503 people of Thakazhy grama panchayat using two stage cluster sampling technique. Data thus obtained was subjected to frequency and percentage analysis along with chi-square goodness of fit test.

From the study it was observed that prevalence rate of amlapitta in Thakazhy grama panchayat was 26.4% with majority of the subjects coming under the age group of 41-60 years. No association was observed between the prevalence of amlapitta and gender of the individual. A diet and lifestyle modification chart was also prepared for the residents of Thakazhy grama panchayat to fulfil the secondary objective.

ABSTRACT

Calculus is dental plaque that has undergone mineralization. In Ayurvedic perspective,dental calculus corresponds to Danthasharkara resulting from drying up of Kapha and Mala by Vata in the oral cavity. Oral unhygienic practices are the chief aetiological factor behind the development of Danthasharkara.The treatment modality adopted was dental scaling which contribute to the increase in associated symptoms as well as some complications ,which is to be taken care after scaling.

The study was designed as an interventional pre and post evaluation without control and the sample size was fixed as 32. Patients diagnosed as having dental calculus within age group 18-60years irrespective of sex were selected from OPD and IPD of Shalakyatantra Govt. Ayurveda College, Trivandrum. The data collection was done using detailed case proforma, relevant laboratory investigations and clinical examinations. Selected patients after dental scaling were given Dantharogasani choorna Prathisarrana followed by Tilayashti taila Gandusha in a sufficient quantity for the next 30 days at morning. Plaque index, Gingival index and symptoms of Dental calculus were assessed prior to dental scaling ,after dental scaling and after treatment.The collected datas were analysed using Wilcoxon Signed Rank test and Paired t test according to the variable. Statistical analysis showed that the treatment modality is effective in managing post scaling complications of Dental calculus.

ABSTRACT

Lower respiratory tract infections remained the most deadly communicable disease, causing 3.0 million deaths worldwide in 2016(WHO).According to Global Burden of Disease, 2015 study, Lower respiratory tract infections (LRTIs) represent 4th most causes of death.

         Agnikumara rasa is a herbal formulation mentioned in Jwara adhikara of Bhaishajya ratnavali. It contains maricha, vacha, kusta, mustha  one part each and vatsanabha, four parts. It has the bhavana dravya of ardraka swarasa. It is indicated in amajwara, kapha jwara, pinasa, prathishaya, agnimandya, sopha, grahani, atisara, sannipatha jwara, kasa and swasa.

        The study has three parts; first is the preparation of three samples of Agnikumara rasa with reference to Bhaishajya Ratnavali jwaradhikara, with variation in the number of bhavana. ie..once, twice and quadruple times of bhavana with ardraka swarasa . Second is the physicochemical analysis of the drug and last is to check its antibacterial activity on selected lower respiratory tract infective bacteria such as staphylococcus aureus, streptococcus pneumoniae and klebsiella pneumoniae. The antibacterial study was done by the well diffusion method and Broth dilution method. It was assessed by measuring the zone of inhibition(ZIH). Triplicates of samples were done & were statistically analysed.

        It was observed that bhavana had enhanced the physico-chemical as well as quantitative parameters of all the three samples. Alcoholic extract of Agnikumara rasa of one bhavana done proved to have antibacterial activity against the three bacteria using agar-well diffusion method compared to other samples( p-value – 0.03, 0.021, 0.27 ). As satisfactory results were not obtained for other samples through this method, the study was repeated with another method, Broth dilution method. All the samples in all the three organisms showed ZIH ranging from 74 to 88 % in Staphylococcus aureus,  78 % to 89 % in Streptococcus pneumoniae, 77 % to 83 % in Klebsiella pneumoniae at 1000 ยตg (1 mg) concentration . All the samples of Agnikumara rasa are found to have antibacterial activity against the three bacteria and the sample with one bhavana is found to have a consistent antibacterial activity against all bacteria when statistically analysed ( two way Anova ,p-value - <0.01) . Bhavana has not reflected any change in antimicrobial property of Agnikumara rasa. Thus, Agnikumara rasa ( B.R Jwaradhikara ) has been proved with antibacterial effect on staphylococcus aureus, streptococcus pneumoniae,klebsiella pneumoniae .

ABSTRACT

Urolithiasis is one of the common disorders of urinary system with a prevalence of 4-20%. Prevalence more in males than female, the problem of stone formation is considered as a medical challenge due to multifactorial aetiology and high rate of recurrence. In modern science treatment modalities includes non-steroidal anti-inflammatory drugs, anti-emetics, and diuretics, in later stages minimum invasive procedures like ESWL, URS, PCNL was done. In Ayurveda urolithiasis comes under Mootrashmari, it was well explained by all acharyas. As per Ayurveda concept  Mootrashmari can treat with Oushadas at initial stage, in later stages can treat with Sastras. The study aims to clinically evaluate combined effect of Shilajathwadi Yoga and Kallurvanhyadi Kashaya in urolithiasis. Shilajathwadi yoga mentioned in Astanga Sangraha Utharasthana Vishopayogika Adhyaya, contains Sudha Shilajathu and Sudha Vatsanabha, for the convenience of the patient it was converted to capsules. Kallurvanchyadi kashaya mentioned in Sahasrayoga kashaya prakarana . After the preparation of the trial drug physicochemical analysis was done. The study was carried out as a single group pre post interventional trail with a sample size of 20. Patients were selected from OPD, Department of Rasasasthra and Bhaishajya Kalpana, Govt. Ayurveda college hospital,  Thiruvananthapuram. The study subjects were administered 500 mg Shilajathwadi Yoga capsule and 48 ml of Kallurvanchyadi Kashaya twice daily before food for 45 days. Assessment was done based on the change in size of stone by Ultrasonography-abdomen and change in score of signs and symptoms before and after treatment. The trial drug was effective in reducing pain, burning micturition, dysuria, renal angle tenderness, number and size of stone with a significance level of  1% (p < 0.01) and haematuria, and pus cells with a significance level of 5% (p < 0.05).

                                                                     ABSTRACT

 Premenstrual syndrome (PMS) is a common cyclic disorder of young and middle aged women characterized by physical, emotional and behavioural symptoms that consistently occur during the luteal phase of the menstrual cycle and resolve quickly at or within a few days of onset of menstruation. The objective of the study was to find out the effect of Sathavari gula in Premenstrual syndrome. The study design was interventional study- pre and post test with a sample size of 20. Females of age group 20-40 years with PMS attending the outpatient department fulfilling the American College of Obstetrics and Gynaecology criteria (ACOG) of PMS were selected for the study. Study tool included ACOG criteria, Case proforma, and symptom assessment by PMS scale. Sathavari gula was given in the dose of 12 g twice daily after food. Administration of drug was started from 7th day of menstrual cycle till the onset of next menstruation and was continued for three consecutive cycles. Participants were reassessed after the completion of 3 months. The outcome variables – Change in levels of Physiological, Psychological and Behavioural symptoms were assessed by PMS scale. Results were analysed statistically. The change in score before and after the treatment was analysed using the Wilcoxon sign rank test. The result showed that Sathavari gula was effective in reducing Physiological,Psychological and Behavioural symptoms of Premenstrual syndrome.

 ABSTRACT

Pregnancy is one of the pertinent factors among the reproductive cycle of every woman. WHO has set about certain guidelines, in the care of pregnant ladies it’s called as Antenatal care. The main aim of this care is to provide an uneventful antenatal period and there by producing a healthy offspring. Yoga provides physical as well as mental wellbeing through a holistic approach. It comprises of Pranayama and Asanas which can bring positive effect on the health status of the pregnant woman as well as results in the good outcome of labor. In our classics garbhini is advised only to avoid daruna anuchitha vyayama and vishama katinasana. The normal viharas for maintenance of pregnancy can be followed. The study design selected was Parallel group experimental design. Participants aged between 18- 30 years registered in Government Ayurveda College Hospital for Women and Children were selected until attaining a sample size of 31 in each group based on their willingness. The data was collected through Case record forum. The participants in the interventional group have given yoga class in the institution during their regular antenatal visits and asked to practice it daily in their residence from 28 weeks of gestation to the first stage of labor.The Outcome of labour was assessed with the help of Partogram, Robson’s class and Apgar score. Antenatal period,  induction, nature of delivery, Robson's class were compared between the groups using a Chi-Squared test. Continuous variables such as total duration of labor (minutes), first stage duration(minutes), second stage duration(minutes), third stage duration(minutes), the time required to attain strong contraction(minutes),  Apgar at 1minute and Apgar at 5 minutes were compared between the groups using an independent t-test.  A p-value less than 0.05 was considered as statistically significant. The interventional group which followed antenatal yoga shows marked reduction in duration of labor, reduced induction rate and less time interval to reach good contraction in active phase. In case of Apgar score at 5 minutes also study group shows much improvement. Hence, we can conclude that antenatal yoga has great role in outcome of labor.
