ABSTRACT

Urolithiasis is formation of calculi anywhere along the course of urinary tract. Urolithiasis can cause post- renal acute kidney injury via obstruction of urinary outflow often set off end stage renal disease (ESRD). Extracorporeal shockwave therapy (ESWT) is the treatment of choice which is not conducive because of side effects (internal bleeding, kidney damage) and high rate of recurrence (about 60% within ten years). There is a scope to find out a medicine which is effective with less side effect. Ashmari is one among the ashta mahagada.So the early diagnosis and proper treatment in proper time is very important in it.In the present study an effort is made to evaluate the Effect of Combination of kshara (Ash) of bark of varuna (Crataeva nurvala Buch – Ham) and yava (Hordeum vulgare Linn) along with guda (Jaggery) and ghrita(Cow’s Ghee) in Urolithiasis. The study drug varunadi kshara yoga is mentioned in Vangasena Samhita Asmari rogaadhikara. Kshara chikitsa is mentioned in treatment of ashmari. Study drug is easy to administer and if proven by clinical trial, drug can be used in the better management of urolithiasis.The trial drugs were subjected to preliminary phytochemical analysis.This study assess the the Effect of Combination of kshara (Ash) of bark of varuna (Crataeva nurvala Buch – Ham) and yava (Hordeum vulgare Linn) along with guda (Jaggery) and ghrita(Cow’s Ghee) in Urolithiasis  in patients with size of stone between 2mm to 10 mm determined by Ultra Sonography of abdomen.Participants of age group of 20 to 65 years were selected as per inclusion criteria and exclusion   criteria. Primary outcome variables size of stone and secondary outcome variable like routine urine analysis (R.B.C, pus cells) and assessment criteria score were analysed before and after treatment. The pre and post data test data will be analysed statistically using paired t test and Wilcoxon ranked t test. Study concludes that the trial drug combination of kshara of bark of varuna (300mg) and yava (150mg) with ghrita(75mg), guda (75mg) administered once daily for 30 days is effective in reducing stone size and symptomatic relief of participants.

                                                                            ABSTRACT

Rheumatoid Arthritis is a chronic inflammatory, systemic auto immune disease characterized by pain, swelling, and stiffness. In India more than 20% of total population is suffering from arthritis. Current medications are prescribed to alleviate symptom of this disease which have associated side effects like heart attacks, stroke, stomach ulcers, bleeding from digestive tracts, kidney damage etc. Hence it is inevitable to find herbal medicines with least side effects. Punarnavadi kwatha is a promising and proven anti arthritic drug using in vivo models. Punarnavadi kwatham along with Pippali choornam is prescribed for Amavastha of rheumatoid arthritis  by many Ayurvedic Physicians in Kerala and showed promising results. Hence present study is an attempt to scientifically validate its effect.

The study design is pre and post interventional, with a sample size of 15 participants with age group 18-60 yrs. Both male and female participants within the age group of 18-60 years with rheumatoid arthritis attending OPD of Govt. Ayurveda College, Thiruvananthapuram will be selected as per inclusion criteria for the study. Participants were given 200ml of bottled kwatha (2 bottles) along with 250mg of Pippali choornam (30 sachets) for 15 days twice daily.  Primary outcome variables DAS 28 score and secondary outcome variables  were analysed on 0th and 16th day. The pre and post test data will be analysed statistically using Paired t-test, Wilcoxson Signed rank test. Study concludes that the trial drug effective in reducing DAS 28 score.
