In Susruta Samhita Sutrastana Misrakadhyaya, drugs have been mentioned for vrana dhoopana like -Sriveshtaka, Sarjarasa, Sarala, Devadaru, Saara vrikshas like asana, khadira. The aim of the study is to evaluate analgesic and antimicrobial effect of this vrana dhoopana yoga in non-healing ulcer. In this modern era, due to sedentary life style and improper dietary habit digestive disorders show greater incidence. Among them  fistula in ano is a trouble shooting one .Fistula in ano  is a track lined by granulation tissue which connects peri anal skin superficially to anal canal , ano rectum or rectum deeply. The main clinical symptoms consists of pain, swelling and pus discharge. Due to higher recurrence rate and post operative complication, fistula-in-ano is a challenge  in current surgical practise.   Bhagandara which is described in Ayurveda classics can be correlated with Fistula in ano. Uthamadi yoga is mentioned in Bhagandara prathishedha adhyayam in Ashtanga hrdaya  and it contains thriphala, khadira, asana and guggulu  .
