DISSERTATION TITLE:PHARMACOLOGICAL ACTIVITY OF BRAHMA RASAYANA IN MYOCARDIAL REPERFUSION INJURY IN H9C2 CELL LINE

ABSTRACT
Myocardial Reperfusion Injury refers to damage of cardiac tissue due to restoration of blood to an oxygen deprived myocardium. This may trigger series of cascades such as inflammation and oxidative stress induced oxidative damage that can induce secondary cardiac dysfunction, including myocardial stunning, no-reflow phenomenon, reperfusion arrhythmias, and lethal reperfusion injury which can paradoxically reduce the beneficial effects of myocardial reperfusion and lead to lethal damage to myocardium by the triggering of apoptosis. This can be tackled with antioxidants. Brahma Rasayana being one such formulation, which is thought to bring a positive result in the adverse effects of reperfusion injuries owing to the presence of antioxidants as ingredients.

The main objective of the study was to evaluate the activity of Brahma Rasayana in improving the regenerative potential in Myocardial Reperfusion Injury through invitro study in H9C2 cell line. The protocols adopted were estimation of antioxidant activity, preconditioning and postconditioning to know the response to addition of ischemic buffer, Mitochondrial Membrane Potential (MMP) and Reactive Oxygen Species (ROS). DPPH scavenging assay was chosen for estimation of antioxidant activity. On DPPH free radical scavenging assay, maximum antioxidant activity was observed at the concentration 200 μg/ml with percentage inhibition 65.4592%. In preconditioning, Brahma Rasayana provided maximum average cell viability of 62.81 % at a concentration 25 μg/ml. In postconditioning, the maximum percentage viability was 93.01% obtained at 100 μg/ml. Also, Brahma Rasayana showed reduction in mitochondrial depolarization and ROS production.

DISSERTATION TITLE:EFFECT OF SODHANANGA SNEHAPANAM WITH DHANWANTARAM GRITHA FOLLOWED BY VIRECHANA IN RHEUMATOID ARTHRITIS

ABSTRACT

Rheumatoid arthritis (RA) is an inflammatory rheumatic disease with progressive course affecting articular and extra-articular structures resulting in pain, disability and mortality. Due to similar presentations Rheumatoid Arthritis can be correlated with Vatarakta mentioned in our classics.Sodhananga snehapanam is a procedure which has a major role in the whole outcome of sodhana. Dhanwantaram gritha is mentioned in Ashtangahridaya Prameha chikitsa which is also indicated for vatasonitha. The objective of this study is to evaluate the effect of Sodhananga snehapanam with Dhanwantaram gritha followed by virechana in Rheumatoid Arthitis. Study design was interventional study with pre-post test without control group. Participants of both sex aged 20-60 years registered in IPD of Government Ayurveda Panchakarma hospital, Poojappura, Thiruvananthapuram diagnosed to have Rheumatoid Arthritis and satisfying inclusion criteria were selected. The base line data were collected through interview, observation and questionnaire. After collecting base line data, detailed history and examination were doneusing clinical case proforma.

A total of 20 participants were selected for the study. They were subjected to consecutivesampling and the intervention was done for 23 days and follow up was done 1 month after the intervention. Deepana pachana was done for 5 days using Amruthotharam kasayam 90ml bd before food and Shaddharana choorna 6g bd with kasayam. Snehapanam with Dhanwantarm gritha was given according to Agni and Koshta of Participants. Snehapana was stopped after attainment samyak snigdha lakshanas followed by abhyanga with pinda tailam and ooshma sveda for 3 days. After that Virechana with Nimbamritadi eranda tailam was administered for 1 day. The participants were assessed before rookshana, before and after snehapana, after virechana and after follow up with regards to changes in subjective parameters.DAS Score, RA factor, ESR, CRP were assessed on 0th, 17th and 47th day. Data will be analyzed according to wilcoxon signed rank test and paired t test. Based on statistical analysis, the study concludes that Sodhananga snehapanam with Dhanwantaram gritha followed by Virechana is effective in reducing pain, tenderness, swelling, morning stiffness in patients of Rheumatoid Arthritis measured by subjective symptomatic grading, Das score and blood parameters such as RA, ESR and CRP.

DISSERTATION TITLE:EFFECT OF DASHAMOOLA KASHAYA NASYA ALONG WITH MOORCHITHA GRITHA AND SAINDHAVA IN THE MANAGEMENT OF MIGRAINE

ABSTRACT

Migraine is a type of primary headache disorder that affects central nervous system causing painful attacks of headache in episodes. According to the prevalence rate, it’s ranked as the third most prevalent disorder in the world and third highest cause of disability worldwide in which females are affected more than males in the age group of 20 to 50 years. In Ayurvedic view, Migraine can be correlated to Ardhavabhedaka, characterized by symptoms such as ardhaparswa sirasoola, periodic attacks and prakashaashishnutha. Nasya is one among the panchakarma procedure, mainly indicated for jathurdhwanga rogas as nasa is the gateway to siras. Even though many nasya formulations have been indicated for Ardhawabhedaka in our classics, now it’s the need of hour, to find out new formulation which can be more effectively used in Ardhavabhedaka. The objective of present study is to evaluate the effect of Dashamoola kashaya nasya along with Moorchitha gritha and Saindhava in reducing the symptoms and recurrence of migraine. The study is an Interventional study with pre post test without control group, done in OPD of Govt Ayurveda college Thiruvananthapuram, with participants of both sexes of age group 20 to 50 years , fulfilling the diagnostic criteria of migraine, selected by consecutive sampling technique as study population, considering the inclusion and exclusion criteria. Nasya using Dashamoola Kashaya along with Moorchitha gritha and Saindhava in madhyama matra is given for 7 days or till samyak nasya lakshna . As poorvakarma, sadhya snehapana and sweda for 3 days followed by virechana with Gandharva erandam given. Assessment were done on 0th day, 5th day, 12thday, 40th day, 68th day with regards to changes in the variables such as intensity of pain, duration of pain, frequency of attack and disability due to migraine with the help of MIDAS questionnaire, VAS scale and clinical case proforma and the data is analyzed by Wilcoxon Signed Rank test. Study results showed a significant reduction in all parameters after the treatment and after the follow up period. The study concludes that Dashamoola Kashaya nasya is effective in reducing the symptoms and recurrence of Migraine.
